Tarsal Tunnel Syndrome
Tarsal tunnel syndrome, which can also be called tibial nerve dysfunction, is an uncommon condition of misfiring peripheral nerves in the foot. The tibial nerve is the peripheral nerve in the leg responsible for sensation and movement of the foot and calf muscles. In tarsal tunnel syndrome, the tibial nerve is damaged, causing problems with movement and feeling in the foot of the affected leg.
Common Cause of Tarsal Tunnel Syndrome
- Involves pressure or an injury, direct pressure on the tibial nerve for an extended period of time, sometimes caused by other body structures close by or near the knee.
- Diseases that damage nerves, including diabetes, may cause tarsal tunnel syndrome.
- At times, tarsal tunnel syndrome can appear without an obvious cause in some cases.
The Effects of Tarsal Tunnel Syndrome

Tarsal Tunnel Syndrome
Tarsal tunnel syndrome is a condition in which there is a compression of the posterior tibial nerve. The posterior tibial nerve runs along the inside of the ankle into the foot. Tarsal tunnel syndrome is named for the tarsal tunnel, which is a thin space along the inside of the ankle beside the ankle bones. This space contains various nerves, arteries, and tendons, and includes the posterior tibial nerve. The tibial nerve is the peripheral nerve in the leg responsible for sensation and movement of the foot and calf muscles. In tarsal tunnel syndrome the tibial nerve is compressed, causing tingling or burning, numbness, and pain.
Common causes of tarsal tunnel syndrome involve pressure or an injury. Injuries that produce inflammation and swelling in or around the tunnel may place pressure on the posterior tibial nerve. Direct pressure on the tibial nerve for an extended period of time, sometimes caused by other body structures close by or trauma to the tibial nerve, can result in tarsal tunnel syndrome. Diseases that damage nerves, such as diabetes or arthritis, may cause tarsal tunnel syndrome. Those with flat feet are at risk for developing the condition, as the extra pressure and strain placed on the foot may compress the posterior tibial nerve.
Feeling different sensations in the foot at different times is a common symptom of tarsal tunnel syndrome. An afflicted person may experience pain, tingling, burning or other unusual sensations in the foot of the affected leg. Symptoms are primarily felt on bottom of the foot and/or the inside of the ankle. Symptoms can appear suddenly and may occur due to overuse of the foot.
To diagnose tarsal tunnel syndrome, your podiatrist may examine the foot and tap the posterior tibial nerve to see if symptoms surface. He or she may also order an MRI to determine if a mass is present.
Treating tarsal tunnel syndrome will depend on the decision of your podiatrist. Multiple options are available, however, and can include rest, ice, immobilization, oral medications such as anti-inflammatory drugs (NSAIDS), physical therapy, injection therapy, orthotics, supportive shoes, braces, and surgery.
Prefabricated Versus Custom Orthotics

Prefabricated orthotics is another name for over-the-counter (OTC) orthotics. Custom orthotics are constructed to the precise specifications of your foot. With certain foot conditions, such as plantar fasciitis or flatfoot, there is evidence that prefabricated or custom orthotics help. They are not considered to help as much for overuse injuries like tendonitis or stress fractures. Rigid, or very stiff orthotics, can help control the foot position during walking but must exert force against the plantar part of the foot that is three to four times one’s body weight. Rigid orthotics are not particularly comfortable either, and less of this type of orthotic is used. Most often, one wears semi-rigid or accommodative orthotics, and they act almost like a bed for the foot spreading the weight more evenly across the surface of the footwear. When there is an abnormality of the foot, custom orthotics are thought to be better. Know that no orthotic—custom or OTC—will be able to treat all foot problems. Ankle Foot Orthotics for Athletes
Ankle foot orthotics are shoe inserts that offer support to control the placement and movement of the ankle, correct deformities, and compensate for weakness. These inserts are used to stabilize the foot and ankle and provide toe clearance during the swing phase of gate.
Athletes often suffer foot problems because their feet are not being supported within the shoe. Ankle and foot orthotics are custom made inserts that alleviate stress on the foot. However custom orthotics should be prescribed by a podiatrist who specializes in customized footwear and orthotics design. These inserts are used by athletes for different reasons. Runners use orthotics to absorb shock at heel contact and to set up the forefoot for push-off. Basketball players wear them to control their forefeet while jumping and running.

While the ankle joint is less likely to be affected by arthritis than other joints, when it is, it means that the tibiotalar joint – the joint between the shin bone and ankle bone – is worn out. The most common cause of this condition is a previous injury, such as an ankle fracture. In such cases, something called osteonecrosis can result from ankle injuries, where there is damage to the blood flow to a portion of the bone that can then lead to ankle arthritis. Other causes of ankle arthritis are rheumatoid arthritis (which can cause joint inflammation and damage to the cartilage), infections of a joint leading to damage of cartilage cells, being overweight, placing a larger burden of weight on the ankle joint, and possibly genetic factors. The most common symptom of ankle arthritis is joint pain. Other indicators of this condition include stiffness in the ankle, swelling in the joint, bone spurs on the joint, deformity of the joint, and a feeling of instability in the joint. One is less apt to feel tingling and numbness in the feet and toes from ankle arthritis due to irritation of the nerves, but it can happen. Changes in footwear, adding cushioned inserts in shoes, using a brace to help hold the ankle in position, and modification of activity can relieve discomfort associated with ankle arthritis. How to Care for Your Arthritic Foot
Arthritis is an inflammation of the joints and it can occur at any joint in the body, especially in the foot. It generally effects those who are older, however, it can occur at any age. Although there are many different forms of arthritis, there are three main types that occur in the foot. The three types are osteoarthritis, rheumatoid arthritis, and gout.
The primary cause of osteoarthritis is aging. As you age, cartilage degenerates around the joints which causes friction and pain. Obesity can cause osteoarthritis through mechanical stress. Injuries that damage joints can increase the probability as well. Finally, a family history of osteoarthritis can also increase chances of having it.
Rheumatoid arthritis occurs when the immune system attacks the joint linings and weakens them over a long time. While there is no known cause of rheumatoid arthritis, obesity and smoking can increase your chances of getting it. Women are also more likely to get it than men.
Gout is a form of arthritis that occurs when there is too much uric acid in your blood and painful crystals form in your joints. Men are more likely to have gout than women. People who are obese or drink alcohol often are also more likely to develop gout. Furthermore, having diabetes, heart disease, high blood pressure, high cholesterol, gastric bypass surgery or a family history of gout may increase your likelihood of developing the condition.
Symptoms of arthritis include pain, stiffness, swelling in the joints. These symptoms can make it harder and more painful to walk. Physical activity can increase pain and discomfort. Furthermore, joint pain can worsen throughout the day for osteoarthritis. Gout attacks generally last several days with the first few being the worst.

Wider shoes can help with discomfort from gout and osteoarthritis. High heels should be avoided. Shoes with proper arch support and that take pressure off the ball of the foot can help with rheumatoid arthritis. Drinking lots of water can also help rid uric acid from the blood. Facts About Peripheral Neuropathy

If you experience a pins-and-needles feeling, a burning sensation, numbness, and tingling, especially in the feet, you may have a condition known as peripheral neuropathy. This is the result of nerve damage that affects the extremities, and is common to diabetics as the result of high blood sugar levels. The risk of peripheral neuropathy increases if you are more than 40 years old, smoke, and drink large amounts of alcohol. A number of other underlying factors can contribute to peripheral neuropathy. Among them are low levels of vitamin B12, underactive thyroid, chronic liver or kidney disease, and inflammation of blood vessels. There is no known cure for peripheral neuropathy, but a number of treatments are available to treat it or lessen its effects. What Is Neuropathy?

The symptoms of neuropathy vary greatly and can be minor such as numbness, sensation loss, prickling, and tingling sensations. More painful symptoms include throbbing, burning, freezing, and sharp pains. The most severe symptoms can be muscle weakness/paralysis, problems with coordination, and falling.
Podiatrists rely upon a full medical history and a neurological examination to diagnose peripheral neuropathy in the foot. The posterior tibial tendon connects the muscle in the back of the shin bone to the bones in the foot. This tendon can become injured due to overuse or trauma and cause pain that is felt on the inside of the ankle. The peroneal tendons connect certain calf muscles to the foot. They can become inflamed at the ankle joint behind the fibula bone due to trauma or overuse and cause swelling and tenderness, along with pain behind the ankle.
